In exchange for attending an event hosted by Gild Collective, LLC, an Ohio limited liability company (the “Company”), I hereby authorize and grant to Gild Collective, LLC, an Ohio Limited Liability Company (the “Company”) the rights to reproduce and use my image, portrait, story, likeness, voice, and related film or picture (collectively, “My Image”), in any work related to the promotion of the Company or its related purpose, which may be in any and all forms or formats, including without limitation all electronic (such as publisher’s website on the Internet), magnetic and digital formats, whether now known or hereafter invented, in perpetuity. . I understand that My Image may be edited, copied, exhibited, published or distributed, or any of the aforementioned in conjunction with other photographs, videos, and/or audio recordings, and I waive the right to inspect or approve the finished product wherein My Image appears. Additionally, I waive any rights I may have to royalties or other compensation arising from or relating to the use of My Image. I hereby specifically release the Company and any individual associated with the Company, including but not limited to any independent contractors or editors related thereto, from any and all liability, including without limitation liability for invasion of privacy or violation of the right of publicity, arising out of or resulting from the reproduction or use of My Image pursuant to the terms hereto. By signing this form, I hereby represent to the Company that I am of at least 18 years of age and have full legal authority and capacity to enter into such agreement.

By signing this consent form, I understand this permission signifies that photographic and/or voice recordings of me may be electronically displayed via the internet or in other public and/or private settings.

By signing this consent form, I understand this permission signifies that I will not be consulted about the use of My Image, and that I will not object to any use the Company, in its sole discretion, decides to put My Image to.

I understand that there is no time limit on the validity of this release, nor is there any geographic limitation on where My Image may be distributed.
